Give us a call! Our calendar only shows our availability for the next 30 days. And we may be able to schedule you sooner!
Clinic Policies
You MUST be a Texas resident to be seen in this clinic.
All initial evaluations are IN PERSON. We can make exceptions to this rule if you live outside the DFW area, so please let us know if this is the case for you!
Follow up appointments can be virtual or in person!
Completion of the appointment request form does not establish a doctor-patient relationship.
Cancellations & Rescheduling
If you are unable to attend your appointment, we ask that you inform Dr. Salako at least 48 hours prior to your scheduled appointment time. You will not be charged a cancellation fee.
If you no-show your appointment or cancel on the same day, you will be charged for the full cost of the appointment.
