Clinic Policies

At Salako Psychiatry, we are committed to providing flexible and personalized care to meet your needs.

You MUST be a Texas resident to be seen in this clinic.

Initial Evaluations:

• Controlled Substance Prescriptions: If you are requesting or require a prescription for a controlled substance, your initial evaluation must be conducted in-person. Dr. Salako reserves the right to deny a medication if she feels it is not right for your condition.

• Non-Controlled Prescriptions: If you do not need a controlled substance prescription, you have the option to choose between a virtual or in-person initial evaluation based on your preference.

• Change in Prescription Needs Post Virtual Evaluation:
  If you complete a virtual initial evaluation and it is determined that you require a controlled substance prescription, your subsequent follow-up appointment will need to be conducted in-person. Additionally, please be aware that controlled substance prescriptions are subject to our discretion and and pharmacy discretion may require an in-person visit before being filled.

  Follow-Up Appointments:

• All follow-up appointments can be scheduled as either virtual or in-person, according to your convenience and preference unless an in-person appointment is necessary.

Completion of the appointment request form does not establish a doctor-patient relationship.

Cancellations & Rescheduling

If you are unable to attend your appointment, we ask that you inform Dr. Salako at least 48 hours prior to your scheduled appointment time. You will not be charged a cancellation fee.

If you no-show your appointment or cancel on the same day, you will be charged for the full cost of the appointment.
